What Is Copay Assistance?

Copay assistance programs are designed to help reduce the out-of-pocket cost burden of specialty and high-cost medications. These programs may be offered by drug manufacturers, nonprofit foundations, or independent assistance organizations. Eligible patients can often access treatments at significantly reduced costs—or even free—by enrolling in the right program.

Types of Copay Assistance

Manufacturer Copay Programs

Offered directly by pharmaceutical companies, these programs reduce the patient’s out-of-pocket cost for branded medications, typically for those with commercial insurance. They often provide copay cards or coupons that cover part or all of the patient's share.

Example: A patient prescribed a $3,000/month specialty drug may use a manufacturer copay card to reduce their monthly payment to $5 or $10.

Nonprofit Foundations

Independent charitable organizations that provide financial assistance for patients with specific diseases or income levels, regardless of insurance status. These funds are often limited and disease-specific.

Example: The PAN Foundation may offer $7,500/year in assistance for patients with multiple sclerosis, applied toward copays, coinsurance, or deductibles.

State and Public Assistance Programs

Government-backed programs such as Medicaid, Extra Help (Low-Income Subsidy), and state pharmacy assistance programs (SPAPs) can lower or eliminate copays for eligible low-income individuals.

Example: A dual-eligible Medicare/Medicaid patient might pay $0 for medications that would otherwise cost hundreds per month.

Appeals & Advocacy Resources

Patients denied coverage or financial assistance can often appeal or seek help from advocacy organizations that assist with paperwork, communication, or benefit navigation.

Example: A patient denied copay support may receive free help from the Patient Advocate Foundation to file an insurance appeal or connect with alternate assistance.
